What to bring to your appointment:
For new patients you will be sent forms to complete by email or SMS and we ask that they are completed prior to your appointment, ideally 2+ days in advance.

You are more than welcome to bring a support person with you to your appointment.
List of things to bring to your first appointment:
- Medication list (what you actually take, not what is listed on the referral)
- List of current supplements/vitamins including brand names
- Any new blood test or radiology reports done after the referral was sent through
- Blood glucose meter (if relevant)
- Blood glucose record book (if relevant)
- Copy of recent Blood Pressure readings (if done)
For patients with Type 1 Diabetes please organise the following (please note sharing of info is not required but is preferred and these steps make it much easier):
- Make sure you are logged into your CGM app (and Clarity for Dexcom users) to allow for sharing with our clinic (we will give you a link code in the appointment to put into your app)
- Ypsopump or Omnipod pump users - bring your Glooko login details (if you don’t have Glooko set up then try to do that before your appointment)
- T-Slim pump users - if you have the Source app/T-Slim app set up please link your account to ‘Mooroolbark Specialist Centre’ prior to your appointment (usually has to be done by logging into the Source app on a browser)
- Medtronic pump users - make sure you’re logged into the Medtronic system. Bring your Medtronic user name and password (that’s the quickest way to link to our accounts)
